The Social Care Network (SCN) is an official New York State Medicaid program. If you have Medicaid in NY State, you may qualify — anywhere in the Hudson Valley.

• Food insecurity or difficulty accessing healthy meals
• Housing instability or unsafe living conditions
• Lack of transportation to medical or social services
• Living with a chronic or qualifying medical condition
• Pregnancy or postpartum (up to 12 months after birth)
• Referred by a healthcare provider, care manager, or social worker
